Situations often arise when it is necessary to obtain not just information about the amount of maternity capital, but also a certificate about the balance of funds on the certificate. You can fill out an application form issued by department employees, or write a request by hand, in free form. In the second option, it should be called: “application for the provision of information on the balance of maternity capital.”
If necessary, assistance in drawing up a written request should be provided by the employee receiving the documents. It must indicate:
